Immunization Letter 7th Grade

Dear Parent/Guardian:

The purpose of this letter is to inform you of immunization requirements that will impact rising 7th-grade students. Students entering 7th grade are now required to have one dose of the Tdap (tetanus, diphtheria, pertussis) vaccine and one dose of meningococcal conjugate vaccine for individuals who have not already received those vaccines.

Please contact your healthcare provider or local health department if your child needs these vaccines. For additional information about the vaccines and diseases they protect against, visit the Centers for Disease Control and Prevention’s website at Vaccines.

Please be prepared, on the first day of school, to provide proof of immunization (i.e., Certificate of Immunization [NC Immunization Report]).

Principals are required by law to prohibit any student from attending school until proof of immunization of required vaccinations has been supplied to the school office. Please be proactive and get your child’s vaccines completed.
